	These are the forms to be used when requesting new access or a change in access. Email Resource Request collects the information necessary to set up an email resource. For more information, please review distribution lists and resources. ImageNow Access Request gathers the information necessary to provide access to the document imaging system. New E-mail/Network Access collects the information necessary to set up email , file access, and computer login information for new employees. For more information, please review r equest new e-mail/network access. PeopleSoft Access Request form gathers the information necessary to provide non-self service access to PeopleSoft systems (Student Administration and Financials). Request for "Friend of DePaul" User ID collects the information necessary to create an ID for external people, temp workers and/or consultants not included on the DePaul payroll who need access to Campus Connection and or Outlook. Update Outlook Addressbook/Exchange Account gathers the information needed to change an employee’s Outlook addressbook information.
